**Major overhaul with stricter consensus validation and new operator-driven configuration.** - Reorganized package structure; all code and documentation moved from `coe-consensus-skill/` to top-level directories. - Operator-supplied consensus policy, validator set, and quorum rules; request-supplied weights/thresholds are now rejected. - Enforces strict validation of signed votes, reference digests, and signer eligibility for each assertion. - Stateless quorum evaluation only; does not persist ledger or execute actions, and re-evaluation does not duplicate votes. - Accepts up to 128 events per request; comprehensive setup, security, and operational guidelines provided. - Shared World State is returned with a JCS SHA-256 hash as integrity check, not as a signed tamper-proof record.
